How to File a Case by Proxy

The File by Proxy function allows authorized managers or investigators to enter a report into IntegrityCounts on behalf of someone who submitted their concern through another channel (for example, via email, phone call or in‑person conversation). Recording these reports centrally helps ensure every concern is captured and investigated through the same workflow.

Who Can Use It
Only designated case managers or investigators should file reports on behalf of others. Always capture the reporter’s words as accurately as possible and respect their anonymity preferences.
 

Step‑by‑Step Instructions

1. Sign in to IntegrityCounts and go to the Case Manager dashboard.

2. In the upper‑right corner, click File by Proxy.

3. In the pop‑up window, click inside the Company field and select the correct company from the drop‑down list.

4. Complete the report form just as the reporter would. Provide details such as location, department, issue type, date of occurrence, and a thorough description of the concern. Use the reporter’s exact language where possible.

5. Attach any files that the reporter provided. Remember that each file must be under 20 MB.

6. Submit the report. The system will generate a case number and temporary password.

7. Write down or securely send the case number and password to the reporter. They can use these credentials to log in, check the status of the case, upload additional information and communicate with investigators.

Tips

  • Ensure you select the correct company before beginning the report.
  • Do not include identifying information if the reporter wishes to remain anonymous.
