How to File a Report in IntegrityCounts

Submitting a report through IntegrityCounts is easy, anonymous (if you choose), and designed to ensure your voice is heard safely and securely. Here’s how to create a case from your side of the platform.

 

1. Visit The Reporting Portal

Go to your organization's IntegrityCounts reporting link (https://www.integritycounts.ca) and click "File a Report" to begin.

 

2. Complete the Form Using Sample Content

You’ll be asked to select:

  • Location of the incident
  • Department involved
  • Your preferred level of anonymity

Anonymity Options:

  • Strictly Anonymous
    Your identity is not known to anyone—not your organization or IntegrityCounts.
  • Anonymous From Your Organization
    IntegrityCounts may use your contact info to send notifications or let you reset your password, but your organization will not see your identity.
  • Contact Information Provided
    You allow a company representative to reach out for follow-up if needed.

 

3. Answer Intake Questions

These help provide context for your report:

Is the issue ongoing?
Choose Yes if the concern is still happening. or No if it is no longer active.

Date of Occurrence
Enter when the event happened to help with the investigation timeline.

Was this reported to a supervisor?
Helps the organization understand if it's been escalated before.

Are you an employee of the organization?
Select Yes or No depending on your relationship (employee, contractor, public, etc.).

 

4. Select the Case Type

Pick up to three categories that best describe the issue. If none apply, use the custom case type field to describe it in your own words.

If the issue doesn’t fall into a listed category, you can enter a custom case type using the "Enter Case Type if it's not included" field.

Then continue by:

  • Describing what happened
  • Listing who was involved
  • Indicating if this issue was referred elsewhere (e.g., police, media)
  • Adding any other relevant details

 

5. Provide Additional Information (Optional)

Upload supporting documents (if applicable)

Protect your report by selecting individuals involved—these users will be restricted from accessing your case

Describe how those individuals were involved: Briefly explain how the selected individuals are involved

 

6. Review and Submit the Report

Double-check your responses. When ready, click Submit.

You’ll be given login details to check back on your case and see any follow-up messages from your organization.

 

Best Practices:

  • Be as specific as possible with dates, locations, and names
  • If uploading files, remove personal information unless it’s relevant
  • Save your case ID and password somewhere secure so you can follow up later
